Qualification

The PSW Coordinator oversees, supports and monitors PSW’s to ensure that direct personal and nursing care fulfil the holistic needs of the resident in accordance with Southbridge’s policies and procedures.  This position also assumes responsibility for the daily operations of the Personal Support Workers (PSWs), which includes ensuring the necessary tools, equipment and resources are provided to PSW’s on all shifts as necessary.

  • Personal Support Worker (PSW) Program Certificate or Diploma that meets the appropriate standards established by the Ministry of Training, Colleges and Universities, the National Association of Career Colleges, or the Ontario Community Support Association. Current enrolment in a nursing program (having successfully completed at least one (1) year is also acceptable Grade
  • Knowledge and experience in care planning systems and documentation required. 
  • Ability to communicate well both verbally and in writing with other staff, residents, and families. 
  • Demonstrated skill in working independently with strong leadership abilities, as well as a valued team player. 
  • Must have a working knowledge of WHMIS and Basic Workplace Safety Training (especially the “Worker’s Responsibilities” under the Occupational Health and Safety Act) as well as the mandatory reporting and whistleblowing policies, and current First Aid and CPR Training. Training in Gentle Persuasive Approach an asset  
  • Must have a current Vulnerable Sector Police Check as well as immunization and vaccination records.
  • Demonstrated record of good attendance.

 

Responsibilities

 

  • Under the direction of the Director of Care, ensure the requirements of Fixing Long Term Care Homes Act (FLTCA) and Regulations, Canadian Council on Health Services Accreditation Standards and Southbridge standards, policies and procedures are met to ensure residents are receiving top-quality care.
  • Adhere to established policies and procedures when observing, reporting and recording electronically information related to resident care and related matters. 
  • Provide input into the development and implementation of policies and procedures which reflect the mission, vision and values of the home 
  • Orientation and mentoring of new PSWs as required. 
  • Provide registered nursing staff with input for the development and maintenance of a current and appropriate plan of care for each resident as identified.
  • Ensure PSW’s follow the plan of care in place for each resident’ 
  • Participate in 100% of all required education programming.
  • Provide education/training to PSW’s, e.g., safe lifts & transfers.
  • Ensure professional communication with the residents, families and other team members 
  • Complete all audits are scheduled and assigned.
  • Completion of reports as indicated and as per schedule.
  • Ensure that PSW’s have the necessary tools to perform their duties/tasks.
  • Participate in all IPAC requirements, while observing IPAC practices by PSW’s.
  • Participate in the Health & Safety Team.
  • Conduct PSW’s interviews, hiring process, and performance management.

 

Quality of Life

 

  • Participates in Continuous Quality Improvement and Risk Management activities 
  • Communicate any progress or decline of a resident to charge nurse or nursing supervisor, or as reported by PSW.  
  • Promote and ensure that residents’ options (what time of the day) and choices (shower, therapeutic tub bath, sponge bath) with hygiene process are provided. – assess resident’s preference as part of audit.  
  • Ensure that the admission, discharge, and transfer of residents’ process is complete as per checklist.
  • Complete audits on documentation in POC including BSO as per schedule. 
  • Assume responsibility for personal/professional growth and development through Staff Development Program, committees, and in-services. 
  • Audit therapeutic tubs as per schedule to ensure they are clean prior to use, that includes observing PSW’s cleaning/disinfecting between use, and they follow cleaning and disinfection protocols to minimize the spread of infection. 
  • Ensure PSW’s utilize equipment and supplies responsibly, and all check lists are completed.  E.g., pre-start up.   
  • Recognize diversity, protects, and support the Residents’ right to their beliefs and values. 
  • Demonstrate respect for co-workers, residents, and visitors in accordance with the Code of Conduct 
  • Contributes to the delivery of person and family-centered care by utilizing communication strategies (person-centered language) that demonstrate compassion, validate emotions, support dignity, and promote understanding.  
  • Audits of units to ensure cleanliness, tidiness to ensure safety and advocate for optimal quality of life for the residents. 
  • Timely reporting of all resident and employee incident/complaints. 
  • Maintains Privacy and Confidentiality at all times.
  • Demonstrate understanding of the residents' Bill of Rights

 

Infection Control & Prevention  

  • Perform all duties in accordance with Infection Prevention and Control standards, policies, and procedures

Customer Service   

  • Ensuring resident privacy
  • Respecting resident’s property
  • Advocating on behalf of the resident 

Environment: 

  • Working in areas where there may involve residents with responsive behavior challenges.
  • Push/pull.
  • Working at/above shoulder height
  • Standing & walking for long periods of time with frequent bending, twisting, reaching, crouching 

Occupational Health and Safety Accountabilities   

  • Carry out all assigned duties in compliance with applicable Occupational Health and Safety legislation and Company policy.
  • Follow safe work practices as directed by their supervisor and as required by the applicable Occupational Health and Safety legislation.
  • Participate in all required occupational health and safety training.
  • Use appropriate equipment and wear personal protective equipment or clothing, as required.
  • Report any known or identified health and safety hazards or violations to their supervisor.
  • Refrain from any conduct that puts themselves, workers, contractors, or others at risk.
  • Ensure the maintenance of a safe environment for residents, staff, and visitors; adhere to expectations that hazardous conditions and actions are promptly corrected and reported.  
  • Report faulty equipment uses Lock Out-Tag Out and ensure immediate removal from the unit and contact charge nurse or nursing supervisor.
  • Utilize equipment with caution and in accordance with the Manufacturer’s Manual(s).
  • Is alert to and properly reports all actual or potential hazardous situations to the immediate supervisor as well as any personal injury to immediate supervisor and seeks first aid as needed
  • Participate in fire safety demonstrations and fire drills and know the facility emergency plan.
