Position: Resident Services Coordinator- Permanent - Full-Time

Duties and Responsibilities

·     Complete resident pre-admission, admission and ongoing social work assessment to identify goals and strategies, expected outcomes for care and target dates.

·     Participate in interdisciplinary team conferences as required.

·     Act as liaison, as appropriate, with families.

·     Provide counselling or group sessions for   residents and  families as identified - i.e., orientation adjustment programs, crisis problem  solving, bereavement counselling, etc..

·     Act as a resource person to staff who have identified needs for counselling, referrals or support.

·     Plan, provide or arrange for in-service programs for facility staff, residents and families as  identified.

·     Maintain all required records, reports, statistics, etc. for both residents and department in  accordance with policies, procedures and legislative requirement.

·     Participate on committees related to resident care and operation of the facility as required.

·     Establishes and maintains communications with other disciplines, residents and families, to  ensure resident needs are being met.

·     Liaises and consults with government officials, company resources and professionals in relation  to resident care needs.

Qualifications

·     Social Services Worker (SSW) Diploma from an accredited College.

·     Bachelor's degree (BSW) in Social Work from an accredited University is an asset

·     Sound knowledge of community resources and services.

·     Sound knowledge in the fields of aging, gerontology and dementia.

·     Previous experience in long term care preferred.

·     Knowledge of applicable legislation, individual counselling and group sessions.

·     Knowledge of occupational health and safety practices, principles and legislation.

·     Ability to read and write English as well as understand complex verbal and written instructions in English.
