This position is presently located at the Woodstock site.
This position is for the Sexual Health Program Team.  
Working within a multi-disciplinary environment, the Public Health Nurse assesses the need for, plans, organizes, develops, delivers, and evaluates health promotion and protection programs and services to enhance the health of the community.
Within provincially mandated public health program standards, Health Unit policies and procedures and College of Nurses standards, the Public Health Nurse works independently as well as collaboratively with internal colleagues and external partners to deliver services to individuals, families, groups, communities and stakeholders for The City of St. Thomas, Elgin and Oxford County.

Qualifications:
EDUCATION

  • At a minimum, successful completion of a Bachelor’s of Science in Nursing (BScN) degree as recognized in the province of Ontario. 
  • Current registration with the College of Nurses of Ontario and eligible to practice in the Province of Ontario.

 

KNOWLEDGE/EXPERIENCE/SKILLS 

  • Knowledge of public health sciences and familiarity with legislation governing public health practices, including, but not limited to the Health Protection and Promotion Act, Immunization of School Pupils Act, Smoke Free Ontario Act.
  • Understanding of social determinants of health (SDOH) and their impact on individual and community health. 
  • Understanding of public health concepts which include population health, epidemiology, health promotion strategies, client-centered care, community engagement, disease and injury prevention, health protection and harm reduction.

  • Demonstrated ability to develop and maintain effective therapeutic relationships with others. 
  • Demonstrated ability to build and maintain credibility with client and community partners. 
  • Demonstrated commitment to professionalism and a high level of integrity. 
  • Confidentiality, tact and discretion when dealing with others. 
  • Effective interpersonal, written and verbal communication skills including group presentation skills. 
  • Excellent organizational, time management and prioritizing skills. 
  • Ability to adapt effectively to changes in workload, policies, procedures and practices of the Health Unit and its governing bodies. 
  • Excellent computer and keyboarding skills as they pertain to documentation systems required for this position. 
  • Ability to apply best practice nursing skills including clinical skills in delivery of programs and services. 
  • Ability to develop and deliver effective public health messages, information, programs and services. 
  • Ability to work independently as well as collaboratively within a multi-disciplinary team of public health professionals. 
  • Demonstrated commitment to a safe and healthy work environment for both employees and clients.
     

SPECIAL CONDITIONS 

  • Valid Ontario driver’s license and reliable access to an insured working vehicle.
  • Compliance with all SWPH’s administrative and relevant program related policies and applicable provincial legislation.

 

Terms and conditions of employment are in accordance with ONA Collective Agreement. 

Salary Range: $36.85/hr to $50.50/hr.
Will receive vacation pay each pay period as well as pay in lieu of benefits following successful completion of the probationary period.  Offered enrollment in the OMERS pension plan (if enrolled, in lieu benefits will reduce). 

Please note: All nurses at SWPH will be expected to remain knowledgeable, skillful, and ready to respond to public health and community-based needs through:

·      Outbreak response efforts (including but not limited to: Collecting and preparing specimens in urgent or emergent situations, case and contact management, etc.)

·      Immunization programs (including seasonal campaigns)

·      Other identified program needs that require nurses to work to their full scope of practice


A criminal reference check completed within the last six (6) months from a Canadian Police Information Centre (CPID), inclusive of vulnerable sector screening, will be required of the successful candidate. 

Immunization Requirements: Fully vaccinated against COVID-19, are able to provide proof of immunization, and acknowledgement to follow Southwestern Public Health's policies and processes related to COVID-19 management in the workplace.
