THE ROLE
As a member of the peopleCare multidisciplinary health care team, the BSO RPN will oversee referrals to the BSO team, and oversee the activities of the Behavioural supports positions in the Home. The RPN will ensure adherence to the respective regulations and legislation in the Long Term Care Act, 2007 and Regulation79/10, the Resident’s Bill of Rights, the standards set by the College of Nurses of Ontario and the home’s mission, vision, values and policies.
Hours of work:
Weekdays: 9am to 5pm
Weekends: 12pm to 8pm
RESPONSIBILITIES
- Adhere to the philosophy of peopleCare BSO teams.
- Provide coaching, consultation and knowledge transfer to staff of the Home, residents and families.
- Support the resident and family by applying evidenced-based learning. Provide information to residents and families on any changes to medication in relation to responsive behaviours.
- Deliver inter-professional mentoring/coaching to members of the health care team to enhance care and capacity. Use P.I.E.C.E.S and other assessments as a framework to determine the plan of care for each individual resident and communicate same in the resident’s plan of care and Kardex. Update this aspect of the residents’ care plan as required.
- Reassess residents in set goal for time frame for effectiveness of plan of care and discharge from program or continuation of program including further evaluation etc.
- Participate in weekly RAP conferences and complete section B and E of the MDS assessment for residents currently on the BSO program.
- Educate and mentor staff using Kardex and on accuracy of documentation.
- Be a member of the Falls committee in the Home and be involved in determining intervention to prevent falls such as medication changes, condition changes such as infection and/or change in cognition.
- Provide a report to the leadership team for risk management.
- Report quarterly to the PAC Committee.
- Make referrals to social worker, recreation/programs, physician, pharmacist as per assessment to increase the multidisciplinary approach to behaviours.
- Participate in the evaluation of the program by staff, health care providers, residents and family members on an annual basis.
- Communicate with families and Substitute Decision Makers.
- When appropriate be part of physicians rounds for residents on your caseload.
- Complete required information from the LHIN and the individual home quarterly or as requested.
- Participate in the training required and offered by the Home to facilitate competencies.
- Use protocols and tools established through the BSO project and the LHIN BSO Action Plan.
- Train fellow employees in behavioural service delivery
- Act as mentors/coaches to caregivers
- Report quality indicators for antipsychotic usage in the home.
- Maintain communication and collaboration among LTC homes to share resources, facilitate knowledge transfer and the spread of best practices.
- Link to system-wide resources for managing complex residents by collaborating with the Schedule 1 hospitals Seniors Mental Health and Addictions Mobile Teams, other specialized supports and the Behavioural Supports LHIN-wide project team.
- Work with all of the above to develop and implement a plan of care that addresses a resident’s responsive behaviours safely and stabilizes the person in the current care setting as much as possible/safely.
- Take responsibility to be “on-call” for High Risk resident.
- Participate in care planning process.
- Complete behavior assessments as required.
EDUCATION AND EXPERIENCE
- Registered Nurse with current Certificate of Registration from the College of Nurses of Ontario, in good standing
- Preference will be given to staff that have the recommended core competencies set out in the LHIN BSO Action Plan.
- Flexible hours of work depending on resident needs currently on the BSO program. Includes all shifts.
- Hours scheduled as provided for in current contract with LHIN.
Other
BSO training is a prerequisite for the job.
