The Peel Youth Wellness Hub, part of the Youth Wellness Hubs Ontario (YWHO) initiative, offers youth aged 12 to 25 a wide range of services, including mental health, addiction, primary care, housing, employment, and more. Our goal is to reduce barriers and promote youth-centered, co-developed care in a welcoming and supportive environment.

Role Overview:

The Youth Wellness Facilitator will serve as the primary point of contact for youth, families, service providers, and community members at the Peel Youth Wellness Hub. This position will support a centralized intake process, utilizing YWHO’s client management systems, and facilitate smooth transitions between services. Reporting to the Youth Wellness Hub Project Lead and working closely with the Trailblazers Youth Centre, the Facilitator will create a welcoming, inclusive, and youth-friendly space that promotes safety and developmentally appropriate support.

Key Responsibilities:

  • Intake & Screening: Lead the intake process using YWHO’s client management systems and standardized screening tools.
  • Youth-Centered Environment: Foster a safe, non-judgmental, and developmentally appropriate environment for youth.
  • Orientation & Support: Serve as the primary point of contact for youth and families, providing orientation and assistance with the client information system.
  • Service Pathways Facilitation: Ensure smooth transitions between services, such as mental health, social supports, and clinical care.
  • Collaboration: Work collaboratively with the Trailblazers Youth Centre team, Youth Advisory Council and external partners to ensure seamless access to services for youth.
  • Engagement & Feedback: Actively engage youth and their families to maintain service engagement and gather feedback to improve program delivery.
  • Alignment with Peel CAS and YWHO Values: Ensure all services align with Peel CAS and YWHO’s youth-centered and inclusive values.
  • Continuous Learning: Participate in the YWHO Provincial Community of Practice to stay updated on best practices and innovations.
  • Flexible Service Delivery: Provide services in-person, off-site, or virtually, adapting to the specific needs of youth.
  • After-Hours Support: Provide after-hours support as needed to ensure comprehensive service coverage.

Qualifications:

  • Education: Diploma or degree in social services, child and youth work, social work or a related field; or an equivalent combination of education and experience.
  • Experience: At least 1 year of experience working in community-based youth mental health, addiction, or social services.
  • Technology Proficiency: Experience with electronic client management systems is an asset.
  • Knowledge of Resources: Strong understanding of community resources and a holistic approach to youth health and wellness.
  • Health Equity Knowledge: Familiarity with anti-oppressive practices, trauma-informed care, cultural safety, Truth and Reconciliation, and 2SLGBTQ+ inclusion.
  • Interpersonal & Organizational Skills: Strong communication and organizational skills, with the ability to adapt to a fast-paced environment.
  • Relationship Building: Proven ability to build trusting relationships with youth, families, and community partners. Experience supporting meaningful participation and embracing diverse perspectives on mental health and substance use challenges.
  • Youth Participation: Commitment to promoting youth involvement, inclusion, and diversity of perspectives in mental health and substance use support.
  • Bilingualism: Fluency in English and French or proficiency in another language is an asset.

Hours of work:

  • 9am-5pm, Monday to Friday
  • Working hours may vary and applicants must be flexible to work outside of standard office hours
