One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.
We are currently seeking a 1.0 FTE permanent Nurse Practitioner, who as a valuable member of the Community Health Centre team will provide Nurse Practitioner assistance to individuals utilizing NorWest CHC substance use programs. The role will include working with an interdisciplinary team to provide a full primary care scope of practice and substance use health teachings for clients of Kwae Kii Win Managed Alcohol Program, Path 525, Safer Supply, and unattached clients of the Rapid Access Addictions Medicine Clinic. Virtual option may be available.
The position has a competitive salary and the Health Care of Ontario Pension Plan is an optional benefit.
Eligible for the Community Commitment Program for Nurses (CCPN) that provides $25,000 in grant funding to eligible nurses in exchange for a 2 year commitment. Click here for eligibility criteria.
Qualifications:
- BScN from a recognized university.
- Certificate in the Extended Class from CNO.
- Broad knowledge, background and interest in the practice of community health, addictions and harm reduction.
- Experience offering Substance Use Health Care and supporting clients across the continuum of care.
- Knowledge of managed alcohol administration is an asset.
- Excellent communication, interpersonal and teaching skills.
- Demonstrated commitment to and knowledge of community-based health care.
- Demonstrated ability to work effectively in a multi-disciplinary team environment.
- Demonstrated experience working across all genders with racialized populations, indigenous peoples, individuals who engage in sex work, individuals who are/have been incarcerated, people with mental health challenges, and people who have experienced trauma.
- Awareness of and sensitivity to health issues particularly within individuals who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured).
