One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.
We are currently seeking 1 FTE Registered Practical Nurse (RPN), who as a valuable member of the Community Health Centres team will triage and provide clinical care, illness prevention and health promotion. The focus will be providing episodic access to virtual care for clients with low acuity health concerns. The Registered Practical Nurse will activate pathways as required referring clients for further consultation and assessments, wrap around supports, and connecting them with services in their community across the North West Region.
The position has a competitive salary and the Health Care of Ontario Pension Plan.
QUALIFICATIONS:
- RPN with current College of Nurses of Ontario registration.
- 3-5 Years Experience as a Registered Practical Nurse.
- Experience providing virtual care.
- Three to five years work experience in a combination of remote rural, hospital and community settings and/or public health along with experience as Registered Practical Nurse providing care to individuals who face multiple barriers to care.
- Awareness of and sensitivity to social, economic and health needs particularly within populations who face barriers to accessing care (under housed, cultural, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured).
- Demonstrated commitment to and knowledge of Northern community-based health care.
- Evening and Weekend work hours are required.
- Demonstrated critical thinking and problem-solving skills, organizational and interpersonal skills with an ability to work effectively and respectfully in a collaborative, inter-professional team environment.
- Demonstrated ability to effectively manage crises in a professional manner.
- Superior communication (verbal and written), interpersonal, and conflict resolution skills.
- High proficiency with electronic medical records and electronic referral systems.
- Proficiency with Microsoft Office applications (e.g., Word).
- Periodic local or regional travel may be required.
- Valid Ontario Driver's license and access to a reliable vehicle.
