One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.
We are currently seeking a compassionate, committed individual to work in a Community Health Care capacity as well as in the Greenstone Rapid Access Addiction Medicine (RAAM) Clinic.
This position will require nursing services within Primary Health Care team, Palliative Care team, and the RAAM team. Within the RAAM clinic, the RN provides addictions care to clients and their families within an interdisciplinary model. The approach to care is strengths-based and rooted in harm reduction and evidence-based practice. Within the Primary Health Care team, this RN will assist in the delivery of primary care services to vulnerable populations through outreach, education, individual support, and screening.
QUALIFICATIONS:
- RN with current College of Nurses of Ontario registration.
- Minimum 3 years nursing experience, ideally in a combination of hospital, community and/or public health settings
- Addictions nursing experience required.
- Ability to work flexible hours including evenings and weekends.
- Strong organizational, communication and leadership skills.
- Ability to work both independently, and collaboratively, within interdisciplinary team environment.
- Perform clinical activities according to the College of Nurses standards for nursing practice.
- Utilizes professional assessment skills to identify physical, mental, and psychosocial needs of clients and families.
- Knowledge and proficiency in current evidence based practices of primary care delivery, with an emphasis on health promotion and harm reduction.
- Experience working in outreach and home settings, flexibility to respond to fluctuating workload and changing environment
- Awareness of and sensitivity to health issues particularly within individuals who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured).
- Valid driver’s license, access to a vehicle, and ability to travel to outreach sites we serve
