About ACE Team

The Adherence and Community Engagement (ACE) Team is a multidisciplinary outreach team dedicated to engaging hard to reach people experiencing health and social inequity. The ACE Team manages a caseload of 35-40 clients, operating 7 days a week, 365 days a year to provide wrap-around health and social supports.

 

About the Role
The Adherence and Community Engagement (ACE) Team is seeking a part-time outreach/peer support worker to assist in the care of Edmonton's marginalized individuals living with homelessness and addiction. The ACE Team is a unique 7 day a week/365 community outreach initiative that uses an intensive case management model to provide daily health and social outreach care. This position entails case management, as part of a team, for 15-20 patients. 2-3 weekday shifts (18 hours/week) would be expected.  Temporary Position  for 6 months.

Key Responsibilities

  • Work collaboratively as part of the ACE health team to connect clients to social support.
  • Support clients in accessing housing (referrals, housing ads, connection to resources).
  • Support clients in attending appointments
  • Work alongside Social Navigator to communicate social care plans and educate other team members to support ongoing work throughout the week.
  • Practices from a trauma informed, culturally sensitive, and harm reduction perspective.

 

Education & Qualifications

  • Police Information Check with Vulnerable Sector
  • Certifications (or willingness to obtain): Standard First Aid and CPR
  • Training in Trauma Informed Care, Cultural Sensitivity, Stigma, and/or Harm Reduction
  • Driver's license is a plus but not required

 

Experience & Skills

  • Minimum 6 months of experience working with marginalized, high risk and vulnerable populations
  • Working knowledge of harm reduction practices, abuse and trauma, sexual exploitation, addictions, mental health, homelessness, inner city, and Indigenous culture
  • Knowledge of and experience with Motivational Interviewing and using evidence-based models of care
  • Expert knowledge of Edmonton community resources for housing, funding, ID, shelter, and basic needs
  • Demonstrated experience de-escalating crisis and potentially violent situations with individuals under the influence of drugs or alcohol, under stress &/or struggling with acute/chronic mental health crises
  • Demonstrated experience completing housing referrals/applications, AISH or other funding applications
  • Demonstrated ability to collaborate with various social and healthcare disciplines to deliver person centered care
  • Comfortable using mobile technology- smartphones and phone applications, mobile laptops, Google Drive, Google Calendar, electronic platforms for client documentation
