About the role
The Aging at Home (AAH) program is a service delivery program to assist seniors 55+ who need assistance to be able to remain living independently in their own home. The heart of the Aging at Home program is delivery of services such as friendly visiting and home supports which includes light house-keeping, meal prep, friendly visiting, laundry and supportive transportation services (grocery, banking, pharmacy). The AAH Coordinator will support access to gas cards for specialized medical appointment calculated based on pre-determined program variables. Included in the duties is the coordination and tracking of contracted services that would provide lawn care and/or snow removal.
What you will be doing
- Ensure that the Aging at Home program is delivered in the spirit and philosophy of the Métis Nation of Ontario’s Statement of Prime Purpose
- Provide client services such as home service or friendly visits, service support assessments and service coordination with outside agencies.
- Coordination and tracking of contracted services that would provide lawn care and/or snow removal.
- Support client to access to gas cards or third party transportation to facilitate access to specialized medical appointment calculated based on pre-determined program variables.
- Assisting clients to access services through referrals.
- Record client services in the Online Health and Access Tracking System (OHATS) and Excel tracking documents.
- Maintain confidentiality at all times, and adhere to privacy legislation
- Financial reporting specific to your program site and budget
- Attend training and meetings outside Métis Nation of Ontario working hours
- Perform Administrative duties, and other duties as instructed by the Program Supervisor
- Participate in on-going professional development activities as time and financial resources permit
- Fulfill all other responsibilities as determined by management of Healing and Wellness Branch
Qualifications:
- Experience working with volunteers in a non-profit environment
- 1 or more years’ experience in client services experience
- 1 or more years’ experience working with elderly population (55+)
- Proficiency with computer programs (Outlook, Excel, Work and PowerPoint)
Preferred:
- Degree or diploma in social services
