Position Description: Reporting to the Program Supervisor, as the Registered Social Worker, you will be working within the context of a collaborative interdisciplinary team reflecting the principles of person-centered and evidence-based care by assisting with coordinating and delivering mental health and addiction services to tenants. You will be supporting these individuals who have experienced long term housing instability and/or long stays in hospitals or in other institutional settings. The focus will be on specifically assisting them in times of transition to/from hospital, incarceration, treatment and intake into Indwell’s program to help them achieve health and housing stability. This position will require a high level of resilience and a willingness to work near challenging situations.

Hours of Work: Full time 40 hours per week, 8 hours a day. Schedule based on the support needs of individuals in the program, including evenings, weekends and public holidays. As the program operates 7 days a week, being on-call for 1 week every 4-6 weeks is required to support the tenants and each other on the team. The on-call rotation consists of carrying a work cell phone 24/7 for that one week, responding to urgent matters.
WHAT YOU WILL BE DOING:
Directly Supporting Tenants
- Skilled in building therapeutic relationships with tenants
- Trauma-informed approach promoting hope and trust while maintaining appropriate professional relationships
- Program screening, intake, substance use assessments, completion, and referrals
- Work individually with tenants to identify and achieve substance related goals as it relates to their individual support needs and recovery
- Work collaboratively with internal and external partners
- Support tenants in issues related to maintaining their health and housing
- Assess complex situations and make appropriate decisions independently and as part of a team
- Confident in utilizing crisis management and intervention skills
- Develop and facilitate relevant group programming to meet the needs of the community
- Knowledge of community resources and provincial addiction treatment service
Support, Direction, and Commitment to Interdisciplinary Team
- Partner with other program staff from multiple disciplines in a participatory, collaborative, and coordinated approach to shared decision-making around case management and referrals
- Medication administration, apartment checks, community events, and other supports as necessary
- Work within a team in on-site, in-home assessments and interventions to support long-term stable housing through effective holistic supports
- Utilizing various asset-based and person-centered frameworks to create and manage care plans with tenants
- Enabling tenants to build capacity to self-manage their health and related concerns, connect tenants to appropriate community and social service supports for health promotion and maintenance, and educate tenants on system navigation and self-advocacy skills and resources within healthcare and community and social services
- Compiling up-to-date resources on community and social services in the GHA and surrounding regions, facilitating information sessions and workshops for tenants on various public health and community and social service topics
Assessment and Documentation
- Participate in Intake process and assessments
- Lead the development of coordinated care planning with internal and external supports
- Complete required documentation in a thorough and timely manner with a focus on maintaining and protecting tenant health records
- Perform administrative tasks as assigned, such as rent collections and financial records
WHAT YOU WILL BRING:
- Bachelor of Social Work (BSW) degree or equivalent
- Registered with a regulated college of professionals or willing to get registered for that discipline
- Experience working with individuals with serious mental illness, addictions, or other special needs
- Knowledge of concurrent disorders and of various addiction treatments/theories is required
- A warm, sensitive, and caring approach, the ability to work within a team, be flexible, organized, responsible, and take initiative. Having strong problem-solving skills, being able to maintain good interpersonal relationships and a genuine interest in supporting people living with mental illness and addictions.
- A harm reduction and trauma-informed lens, seeking to uphold the health and wellness of individuals as well as the community
- Experience and ongoing learning around supporting diverse individuals and communities (i.e. cultural safety, anti-racist and anti-oppressive practice, etc.)
- Strong leadership skills, critical thinking abilities and has a passion for mentoring others
- A valid “G” class driver’s license with own vehicle
